Case Management Services
Jail case management services support individuals during incarceration and throughout the transition back into the community. Case managers assess needs, develop individualized plans, and provide reentry navigation, including referrals to housing, transportation, behavioral health treatment, substance use services, employment resources, and other social supports.
They coordinate care to ensure continuity of services—including medical, mental health, and MOUD treatment—so that care initiated in the jail continues seamlessly after release. Case managers serve as an additional layer of care, collaborating with community providers and justice system partners to deliver wraparound support that reduces recidivism, enhances the safety and stability of jail operations, strengthens community safety, and promotes public health outcomes.
