Obtaining Medical Examiner Reports/Records

How can I get a copy of a death certificate?
 
Certified and non-certified copies of death certificates can be obtained from the Jackson County Register of Deeds Office located at the courthouse.  A Funeral Director may order multiple copies of the death certificate from the Register of Deeds Office on behalf of a client family.  The Medical Examiner's Office cannot reproduce copies of the death certificate.

Pursuant to Wis. Stats. 19.34(1), this Notice is provided to the public as to times, place and methods whereby the public may gain access for the purpose of inspection and copying of public records maintained by the Jackson County Coroner's Office.

  • TYPES OF RECORDS

              Records pertinent to the investigation of death, including but not limited to:  Medical Examiner reports, autopsy reports, toxicology reports, autopsy, cremation, disinterment and embalming permits. 

              Some records access may be limited to protect confidential medical information or the integrity of an ongoing investigation.

  • ESTABLISHED TIMES FOR ACCESS:

              8:00 a.m. to 4:30 p.m. Monday through Friday (by appointment)

  • METHOD OF ACCESS:

              Requests for records should be made to the Jackson County Medical Examiner.  Each request should reasonably describe the type of record or information requested.  Identification of requester shall be required as provided by law.  Requesters may be required to review the records in the presence of the custodian.  It shall be the responsibility of the requester to abide by and comply with all regulations and restrictions upon access to or use of information specifically prescribed by law. As soon as is practical, the custodian shall notify requesters of partial or complete denial of access.  Oral requests may be denied orally. If the requester submits a written demand for a written statement of reasons for the denial within five business days of the oral denial, the custodian shall provide such written statement.

  • COPYING OF RECORDS, COST OF COPIES AND RESEARCH:

              Requesters of records or information shall ask the custodian to provide copies of the same, which shall be provided at the established rates per copy.  Prepayment will be required if the total amount of the fees exceeds $5.00.  In the event that the cost, in terms of labor and of expenses, of locating a record exceeds $50.00, the requester shall be assessed such costs.  In addition, the requester shall pay the actual costs of mailing and shipping.

                       Printed to glossy photo quality paper:  at reproduction costs

                       Copied to CD:  at reproduction cost                 
     3. Histology slides and tissue blocks:  at reproduction cost determined by pathologist/laboratory service.
