Reentry Navigator – Job Description

Jail Division

Pay Grade: 10 (Contingent on Grant Funding)

FLSA Status: Non-Exempt

Reports To: Jail Administration, Grant Administration, and Grant Source

Position Summary:

The Reentry Navigator provides comprehensive, person-centered reentry services for individuals incarcerated in the Jackson County Jail, to include individuals with opioid use disorder (OUD), other substance use disorders (SUD), co-occurring mental health needs, or overdose risk factors. The Navigator conducts pre-release assessments, develops individualized reentry plans, facilitates warm handoffs, and coordinates access to  medications for opioid use disorder (MOUD). This role includes overdose prevention education, naloxone distribution, and collaboration with jail, medical, and community partners.

Essential Duties and Responsibilities:

  • Identify and engage eligible individuals using booking screenings, medical referrals, staff referrals, and self-referrals.
  • Conduct needs assessments and develop individualized reentry plans.
  • Coordinate MOUD initiation or continuation with jail mental health, medical staff, and contracted providers.
  • Schedule appointments with community-based treatment and recovery providers.
  • Facilitate access to housing, transportation, public benefits, peer support, and support services.
  • Provide overdose prevention education and distribute naloxone kits.
  • Implement warm handoff and follow-up protocols during the high-risk post-release period.
  • Collaborate with jail staff, jail mental health services, medical providers, behavioral health partners, and public health agencies.
  • Maintain secure documentation including assessments, service plans, referrals, and outcome data.
  • Ensure culturally responsive, trauma-informed practices including motivational interviewing and strengths-based engagement.
  • Participate in multidisciplinary teams and continuous quality improvement processes.

Minimum Qualifications:

  • Bachelor’s degree in social work, psychology, public health, criminal justice, or related field.
  • Two or more years of experience providing case management or reentry services.
  • Knowledge of OUD/SUD treatment, MOUD, harm reduction, and overdose prevention.
  • Training or experience in trauma-informed care and motivational interviewing.
  • Valid driver’s license and ability to pass background checks.
  • Understanding criminogenic needs and how to effectively implement appropriate methods to reduce recidivism.

Preferred Qualifications:

  • Relevant licensure or certification (CSW & SAC)
  • Experience coordinating MOUD across custody and community settings.
  • Familiarity with electronic case management systems and grant reporting requirements.

Work Environment and Physical Requirements:

Work is performed in a secure correctional environment and community settings. The role requires periods of sitting, standing, walking, and travel within Jackson and surrounding counties. Must comply with all Sheriff’s Office jail safety and security protocols.

Compensation and Benefits:

Target starting wage is $31.50 per hour, consistent with grant funding and county wage structure grade 10. This is a full-time project employee specifically grant-funded position. Sustainability contingent on funding source.
