Community Support Program
A team of mental health professionals works with individuals
with serious and persistent mental illness who live in the community. The team identifies needs and provides or
arranges services to meet those needs to increase symptom stability and to
support the participants’ ability to remain in the community.
Who is CSP for?
Jackson County residents who have a serious and persistent
mental illness, who are having difficulty coping with daily living and ordinary
demands of life, and who wish to continue living in an independent or community
setting.
What is CSP?
The Community Support Program provides individualized,
effective and easily accessible treatment, rehabilitation and support services
in the community to enhance participant’s functioning and independence.
What services are provided?
CSP provides or makes arrangements for provision of the
following services:
Comprehensive assessment and treatment planning
Treatment
Crisis
intervention
Symptom
management
Supportive counseling
Medical
prescription
Medical
monitoring
Psychotherapy
Rehabilitation/Skill Therapy
Employment/vocational
activities
Daily
living activities
Social and
recreational activities
Case Management
Coordination
and monitoring of treatment services
Ongoing
review of treatment needs
Advocacy
for needed benefits and services
Education
and support to participants’ family
How are referrals made?
Anyone can make a referral to CSP, including professionals,
concerned friends and family or the individual seeking services. Unless committed for treatment through a
court order, participation in the program is voluntary.
Is there a cost?
Medical Assistance, when available, will be billed for
services. Participants are not
currently billed for services. CSP is
funded through the Jackson County Department of Health and Human Services.
Program Information
A team consisting of a psychiatrist, case managers, nurse
and mental heath technicians provides services.
CSP office hours are scheduled Monday-Friday from 8:00 a.m.
to 4:30 p.m. Hours can be extended to
meet individual participant’s needs.
Staff can be contacted for after –hours and weekend emergencies through
an on-call service.
